What happened to a 44 year old woman who had a history of provoked deep vein thrombosis

A 44-year-old woman had a history of provoked deep vein thrombosis that had been treated with warfarin for 3 months. Shortly after warfarin was discontinued, she developed recurrent unprovoked pulmonary embolism and was started on therapeutic doses of rivaroxaban. Coincident with initiating rivaroxaban, she reported HMB. She described passing blood clots associated with abdominal cramps. An earlier pelvic ultrasound had revealed no structural abnormality. On 1 occasion, she required hospital admission and packed red blood cell transfusion because of symptomatic anemia.

What is the treatment for acute HMB?

In most patients with acute HMB, specific hormonal therapy, including high-dose intravenous or oral estrogen, high-dose progestin, or COCs, is usually needed along with anticoagulant management to control bleeding.

Is anticoagulant bleeding considered a problem?

Anticoagulant-associated heavy menstrual bleeding (HMB) is an underrecognized but not uncommon problem in clinical practice. Premenopausal women should be advised of the potential effect of anticoagulant therapy on menstrual bleeding at the time of treatment initiation. Consequences of HMB should be assessed and treated on an ongoing basis.

Is thrombosis a risk factor for anticoagulant therapy?

The risk of thrombosis during anticoagulant therapy with these treatments is not well studied but is likely to be low. Selection of type of hormonal therapy is based on patient preference, other indications for and contraindications to therapy, adverse effect profile, and ongoing thrombotic risk factors.

Can endometrial ablation be used for HMB?

Endometrial ablation has been widely used in healthy women with HMB who are refractory to hormonal therapy and do not wish to preserve their fertility. Small studies have demonstrated that hysteroscopic endometrial ablation to avoid hysterectomy in women who receive anticoagulants resulted in a reduction in menstrual bleeding (or amenorrhea) in 80% of them. 66, 67

Does unfractionated heparin cause menstrual bleeding?

Unfractionated heparin/LMWH. In theory, unfractionated heparin (UFH)/LMWH should have an effect on menstrual bleeding similar to that of other anticoagulants. However, no study has reported or specifically investigated the effect of UFH/LMWH on menstrual bleeding.

Does a cyclical contraceptive increase hemoglobin?

Although not studied in women who receive anticoagulants, the use of cyclical combined oral contraceptives (COCs) significantly decreases menstrual blood loss and increases hemoglobin levels in women with HMB. 32-34 Combined contraceptive patches or vaginal rings have not been specifically studied in women with HMB.

Can I take birth control during my period?

Q: Birth control helped with my periods, and now I can’t take it because of blood clots.

Can you have a heavy period on blood thinners?

Not every woman has a heavy cycle on blood thinners, so if you do, you need to discuss it with your doctor to find out if it is normal or not. A possible side effect of the newer oral anticoagulants are heavier periods, so if you take those medications, talk to your doctor about your concerns and what to expect.

Can anticoagulants cause a heavy cycle?

It can be difficult to manage a heavy menstrual cycle on anticoagulants, and for many women, it can be uncomfortable to talk about, but it is really important. Not only can an unusually heavy cycle be dangerous to your health, it can be damaging for your self-esteem. It can even effect work and social commitments.

What is the treatment for amenorrhea?

Mainstays of treatment include hormone therapies such as the levonorgestrel intrauterine system, subdermal implant, and other progesterone-based therapies, which can result in decreased blood loss and, in some cases, amenorrhea.

What is heavy bleeding?

Heavy menstrual bleeding (HMB) is defined as menstrual blood loss (MBL) of >80 mL per cycle. 1 For research purposes, the Pictorial Blood Loss Assessment Chart, a chart with which patients can report the number of hygiene products used and the degree of saturation throughout a menstrual cycle, is a commonly used and accurate diagnostic tool with a sensitivity and specificity of >80% for scores >100. 2 Although this tool is also available in the clinical setting, administration may not be practical, and therefore many clinicians must rely on a snapshot history and physical examination in combination with laboratory findings.

What is the treatment for HMB?

An important but often forgotten aspect of the care of patients with HMB is treatment of iron deficiency anemia. All women reporting HMB should be tested with a CBC and a ferritin level measurement. Iron therapy, either oral or intravenous, should be administered as necessary.

Why is hormone therapy important?

In addition to being effective for the reduction of bleeding, hormone therapies provide contraception, which is particularly important in women receiving teratogenic agents, such as warfarin, and in women with a recent history of VTE due to the dramatically increased risk of recurrence with pregnancy.

Can anticoagulation cause postmenopausal bleeding?

Patients receiving anticoagulation are vulnerable to all the same gynecological causes of bleeding as non-anticoagulated patients, and therefore the possibility of underlying causes beyond anticoagulation must be considered. Postmenopausal bleed ing is worrisome for a potential diagnosis of endometrial cancer, and periods that are both heavy and painful can be indicative of other problems, such as fibroids or endometriosis, which may improve with targeted management strategies offered by gynecologists.
