Legacy accepts most health insurance plans including HMO/PPOS, Medicare and the following CHIP and Medicaid plans: Amerigroup, CHIP, Community Health Choice, Medicaid, Molina Health Care, Superior Health Care, Texas Children’s Health Plan and United Healthcare. Our history

Our roots in serving the community run deep. Good Samaritan Hospital was founded in 1875 by the Episcopal Church; Emanuel Hospital in 1912 by the Lutheran Church — each hospital began with a mission to care for the community. In 1989 they joined with Mount Hood Hospital (opened 1922) and Meridian Park Hospital (opened 1973) to form Legacy Health.

The heart of our care: nurses

When it comes to caring, our nurses are front and center. Some 5,000 strong, our nurses occupy a variety of roles across Legacy –– from the patient’s bedside to our electronic information technology team to leadership positions.

Credentialing Services

Legacy's System-wide Credentialing Verification Services (SCVS) personnel perform all of the primary source verification for all health care practitioners within Legacy Health. This information is then forwarded to Medical Staff Services personnel located at each facility to complete the credentialing and privilege reviews for membership and/or privileges.

New Applicants: What You Should Know

If you are a new medical staff applicant, a credentialing specialist with the SCVS will assist you with the credentialing application process, including providing access to your Practitioner Home Page (PHP).
