When should thrombocytopenia be corrected?

People with mild thrombocytopenia might not need treatment. For people who do need treatment for thrombocytopenia, treatment depends on its cause and how severe it is. If your thrombocytopenia is caused by an underlying condition or a medication, addressing that cause might cure it.

When should I start steroids with ITP?

We suggest: Treatment be administered to for newly diagnosed patients with a platelet count <30 × 109/l (grade 2C); Longer courses of steroids are preferred over shorter courses of corticosteroids or IVIg as first-line treatment (grade 2B);

What is the first-line treatment for ITP?

The standard initial treatment for ITP is oral corticosteroids to increase platelet counts. Intravenous immunoglobulin or anti-D immunoglobulin can also increase platelet counts and are particularly useful for stimulating rapid platelet increases before planned procedures.

Should ITP be treated?

Children usually improve without treatment. Most adults with ITP will eventually need treatment, as the condition often becomes severe or long term (chronic). IVIg is generally recommended for ITP patients under critical bleeding condition, as an emergency rescue procedure, and even for those not responding to GCs or cannot tolerate glucocorticoids (6,23,33).

How is ITP 2021 treated?

In adults with newly diagnosed ITP, the ASH guideline panel recommends against a prolonged course (>6 weeks) of prednisone in favor of a short course (≤6 weeks) and suggests either prednisone (0.5 - 2.0 mg/kg/day) or dexamethasone (40 mg/day for 4 days) as the type of corticosteroid for initial therapy1 .

What happens if ITP is not treated?

The specific symptoms of ITP are affected by your platelet count: The lower your count, the more likely you are to have spontaneous and unexpected bleeding, both internally and externally. Without treatment to correct platelet counts, bleeding can become severe and life threatening.

ITP may be acute and resolve in less than 6 months, or chronic and last longer than 6 months. Treatment options include a variety of medications that can reduce the destruction of platelets or increase their production. In some cases, surgery to remove the spleen is necessary.

Can ITP resolve itself?
