To the left of Intensive Treatment is a wall you can jump over, follow the route to the Batcave. From the batcave, look at the map and follow it to a very long horizontal corridor. When you get there, there will be water of the floor, and you'll have to use the Line Launcher to get across the water into Intensive Treatment.

In the Transfer Loop, search for a grate up high in the eastern area (use Detective Mode to reveal it in orange). It leads into the center of the tall shaft at this end of the area. Pull it down with the Batclaw when you get it and grapple up. Follow the shaft back to the marker.
