Inclusion criteria for the population (s) of interest should be defined in terms of relevant demographic variables, disease variables (i.e., variations in diagnostic criteria, disease stage, type, or severity), risk factors for disease, cointerventions, and coexisting conditions. 18 For example, if an SR is focusing only on adult populations, then the inclusion criteria should specify the age range of interest. Ambiguity in population inclusion criteria increases the risk that inclusion decisions could be influenced by differing viewpoints about potential relationships between particular demographic or disease factors and outcome. Table 2 illustrates one such example of how inadequate description of inclusion criteria for a heart failure population may bias the results of SR. Inclusion criteria for population subgroups of interest should also be defined with similar specificity.

Reviewers compare decisions and resolve differences through discussion, consulting a third party when consensus cannot be reached. The third party should be an experienced senior reviewer. The two stages of assessment are discussed in more detail below. Dual review can help identify misunderstandings of the criteria and resolve them such that the studies included will truly fulfill the intended criteria.

Why is selection bias important?

One of the reasons we are concerned about selection bias is because it gives the researchers substantial room for judgment calls in their choice of comparison group. When it comes to studies on non-profits' impacts, we believe that researchers generally prefer to present the programs in a positive light, and thus tend to choose comparisons that favor the programs (more on this below under "Publication bias"). Thus, we feel that selection bias is generally likely to skew apparent results in favor of non-profits' programs.

One source defines quality as “the extent to which all aspects of a study’s design and conduct can be shown to protect against systematic bias, nonsystematic bias, and inferential error.” 4 The Grading of Recommendations Assessment, Development and Evaluation Working Group (GRADE) uses the term quality to refer to an individual study and judgments based about the strength of the body of evidence (quality of evidence). 5 The U.S. Preventive Services Task Force (USPSTF) equates quality with internal validity and classifies individual studies first according to a hierarchy of study design and then by individual criteria that vary by type of study. 6 In contrast, the Cochrane collaboration argues for wider use of the phrase “risk of bias” instead of “quality,” reasoning that “an emphasis on risk of bias overcomes ambiguity between the quality of reporting and the quality of the underlying research (although does not overcome the problem of having to rely on reports to assess the underlying research).” 1

What is the purpose of the systematic review of CQ and HCQ?

The aims of this systematic review are to systematically identify and collate 24 studies describing the use of CQ and HCQ in human clinical trials and to provide a detailed synthesis of evidence of its efficacy and safety. Of clinical trials, 100% showed no significant difference in the probability of viral transmission or clearance in prophylaxis or therapy, respectively, compared to the control group. Among observational studies employing an endpoint specific to efficacy, 58% concurred with the finding of no significant difference in the attainment of outcomes. Three-fifths of clinical trials and half of observational studies examining an indicator unique to drug safety discovered a higher probability of adverse events in those treated patients suspected of, and diagnosed with, COVID-19. Of the total papers focusing on cardiac side-effects, 44% found a greater incidence of QTc prolongation and/or arrhythmias, 44% found no evidence of a significant difference, and 11% mixed results. The strongest available evidence points towards the inefficacy of CQ and HCQ in prophylaxis or in the treatment of hospitalised COVID-19 patients.

What is IAPT evaluation?

The evaluation of demonstration sites set up to provide improved access to psychological therapies (IAPT) comprised the study of all people identified as having common mental health problems (CMHP), those referred to the IAPT service, and a sample of attenders studied in-depth. Information technology makes it feasible to link practice, hospital and IAPT clinic data to evaluate the representativeness of these samples. However, researchers do not have permission to browse and link these data without the patients' consent. To demonstrate the use of a mixed deterministic-probabilistic method of secure and private record linkage (SAPREL)--to describe selection bias in subjects chosen for in-depth evaluation. We extracted, pseudonymised and used fuzzy logic to link multiple health records without the researcher knowing the patient's identity. The method can be characterised as a three party protocol mainly using deterministic algorithms with dynamic linking strategies; though incorporating some elements of probabilistic linkage. Within the data providers' safe haven we extracted: Demographic data, hospital utilisation and IAPT clinic data; converted post code to index of multiple deprivation (IMD); and identified people with CMHP. We contrasted the age, gender, ethnicity and IMD for the in-depth evaluation sample with people referred to IAPT, use hospital services, and the population as a whole. The in IAPT-in-depth group had a mean age of 43.1 years; CI: 41.0-45.2 (n=166); the IAPT-referred 40.2 years; CI: 39.4-40.9 (n=1118); and those with CMHP 43.6 years SEM 0.15. (n=12210). Whilst around 67% of those with a CMHP were women, compared to 70% of those referred to IAPT, and 75% of those subject to in-depth evaluation (Chi square p<0.001). The mean IMD score for the in-depth evaluation group was 36.6; CI: 34.2-38.9; (n=166); of those referred to IAPT 38.7; CI: 37.9-39.6; (n=1117); and of people with CMHP 37.6; CI 37.3-37.9; (n=12143). The sample studied in-depth were older, more likely female, and less deprived than people with CMHP, and fewer had recorded ethnic minority status. Anonymous linkage using SAPREL provides insight into the representativeness of a study population and possible adjustment for selection bias.

Experience Sampling Method (ESM) is a structured diary technique assessing variations in thoughts, mood, and psychiatric symptoms in everyday life . Research has provided ample evidence for the efficacy of the use of ESM in general psychiatry but its use in forensic psychiatry has been limited. Twenty forensic psychiatric patients participated. The PsyMate™ Device emitted a signal 10 times a day on six consecutive days, at unpredictable moments. After each “beep,” the patients completed ESM forms assessing current context, thoughts, positive and negative affect, and psychotic experiences. Stress was measured using the average scores of the stress related items. Compliance rate was high (85% beeps responded). Activity stress was related to more negative affect, lower positive affect, and more psychotic symptoms. This finding was restricted to moments when a team member was present; not when patients were alone or with other patients. ESM can be useful in forensic psychiatry and give insights into the relationships between symptoms and mood in different contexts. In this study activity-related stress was contextualized. These findings can be used to personalize interventions.
